Innovative solutions for COPD multi-morbid patients
To help Chronic Obstructive Pulmonary Disease (COPD) patients with comorbidities and to reduce the burden on our health systems, the WELCOME project aims to create innovative solutions such as an integrated care management tool and a monitoring vest.Great attention will be paid to the small-scale validation of the project and its impact on healthcare in five countries (Greece, UK, Ireland, Germany and the Netherlands).
