Harmonization of the hospital systems - a clinical pattern of the cross-border home healthcare provided for the people residing in the Veneto region and in the Obalno-Kraska area in Slovenia

The cross-border cooperation project aims at a deeper knowledge of the mutual administrative and regulatory systems to detect the most suitable joint actions that can promote exchanges of medical and nursing skills, remote healthcare, the direct intervention of medical staff and the patients’ mobility between the two hospitals. The clinical pattern herein proposed is meant to integrate the work of health facilities staff with the hospital departments through the use of new technologies, so that patients can benefit from a direct home assistance. The project involves general practitioners, home health aides, the specialist nurses of local health district and the health staff working in the departments of the hospital control centres. The clinical programme includes chronic patients, who require a continuous around-the-clock monitoring of some vital parameters, or patients who are cared for by healthcare providers at regular intervals. The pattern envisages the recording of the patients’ vital parameters, the telematic data transmission to the data monitoring centres, the physician’s consultation and possibly the transmission of results to the local operators and the general practitioners. To implement this system, some guidelines have been drawn up for the training of the hospital staff. Thanks to the use of the Internet, videoconferences and e-learning, fast and innovative exchanges are possible. The project aims at providing a quality-certified home healthcare, whereby the service is organized in keeping with a common health protocol consisting of several functional areas, that must be interrelated.
